Our approach to social care
We are on a transformational journey and aim to set a new benchmark for the delivery of social care. We’ve listened to what Ceredigion’s children, young people and adults need to be better able to respond to challenges and build resilience. The focus of our Through Age Wellbeing model is to make sure that all citizens have access to a range of health and social care related programmes and interventions, targeting those at greatest risk.
We are moving away from focusing on the individual and their challenges in isolation to take a more holistic approach.
This means strengthening families so that children and young people can safely remain with their family and enabling adults to live independently in their own community.
Crucially, and most importantly, this also means that we are better able to target our support to those at risk of abuse, harm and neglect.

We provide the least intrusive response to risk, offer support in a timely manner and only with aspects of a person’s life where support is needed.
