Our approach to social care
We are on a transformational journey and aim to set a new benchmark for the delivery of social care. We’ve listened to what Ceredigion’s children, young people and adults need to be better able to respond to challenges and build resilience. The focus of our Through Age Wellbeing model is to make sure that all citizens have access to a range of health and social care related programmes and interventions, targeting those at greatest risk.
We are moving away from focusing on the individual and their challenges in isolation to take a more holistic approach.
This means strengthening families so that children and young people can safely remain with their family and enabling adults to live independently in their own community.
Crucially, and most importantly, this also means that we are better able to target our support to those at risk of abuse, harm and neglect.

We support people to make choices and decisions that are meaningful to them, and help them to express who they are and what they want to be different in their lives. -

We support people with their independence resilience, and help them develop the skills, knowledge and resources to achieve their aims. -

We empower people to have freedom of choice and to make their own decisions - from how they want to be cared for, to how they wish to spend their free time. -

We work with people so that they're actively involved in managing their wellbeing and achieving their goals, and making decisions about their care. -

We provide the least intrusive response to risk, offer support in a timely manner and only with aspects of a person’s life where support is needed.
