Location: Various locations throughout Ceredigion

Hours: Full time, 37 hours per week
All roles have a £2.4k market supplement (paid £200 p/m)
Salaries (per annum):
Newly Qualified: £31,099 - £32,909
Social Worker: £34,723 - £36,298
Senior Practitioner: £36,371 - £38,553

Job Purpose:
Undertake statutory duties according to the SSWBA, focusing on assessing, supporting, and safeguarding vulnerable individuals. Work within legal frameworks and Council policies to assess, commission, and review support services. Collaborate with colleagues across sectors to empower service users while ensuring their safety.
Overview of Through Age and Wellbeing Integrated Services Model:
This model ensures timely, tailored support by being the initial point of contact, designing bespoke prevention and intervention packages, selecting appropriate services, and assessing their impact.
Duties and Responsibilities:
Operational Delivery:
Assist individuals and families in resolving daily difficulties.
Conduct investigations and interventions to protect vulnerable individuals.
Ensure safety through legal processes, providing court statements and reports.
Collaborate with individuals, professionals, and agencies using the Signs of Safety model.
Provide information, advice, and advocacy to service users and carers.
Develop and review care plans within legal frameworks.
Maintain knowledge of regulations and practice.
Foster close working relationships with agencies and contribute to training.
Quality Assurance:
Ensure support meets assessed needs.
Evaluate service user satisfaction.
Develop risk management and safeguarding plans.
Maintain accurate records.
General Responsibilities:
Develop skills and meet standards under guidance.
Promote Welsh language and service user preferences.
Foster partnership working.
Promote dignity in care and safeguarding.
Adhere to statutory legislation and internal procedures.
Adapt duties to service and community needs.
Attend meetings, supervision, and training sessions.
Person Specification:
Essential:
Degree in social work or equivalent.
Welsh and English language skills.
Registration with Social Care Wales.
Ability to communicate effectively.
Understanding of relevant legislation and procedures.
Experience in multi-agency working.
Desirable:
Ability to work in a team.
Effective remote working skills.
